Job Description

The University of Nevada, Reno School of Medicine invites applications for a Licensed Clinical Social Worker to provide services in the Sanford Geriatrics Specialty Clinic.

The Coordinator of Clinical Social Services (LCSW) provides individual services to clients and families of the Sanford Geriatrics Specialty Clinic (SCGC) at the Sanford Center for Aging (SCA).Key services to be delivered include: team-based Comprehensive Geriatrics Assessments (CGA), Chronic Care Management (CCM), and Behavioral Health Consults (BHC). The incumbent works with clients and families to conduct psychosocial and health risk assessments, develop treatment and care plans, complete client intake and follow-up surveys, and make recommendations for community-based long-term support services.

 

The incumbent Licensed Clinical Social Worker will provide direct clinical service, including psychosocial assessments, completion of social work care plans, community support referrals, and care for patients in the Telemedicine Program. Additional duties include developing the psychosocial behavioral health program and services to be delivered in person an via telemedicine technology, collaboration with entities within the School of Medicine regarding documentation, billing, and coding for services provided, and identification of other implementation needs for delivery of psychosocial behavioral health services.

Required Qualifications

Master's Degree and two years of relevant work experience

Relevant work experience: social work, senior outreach, chronic care management, working with older adults and persons with disabilities.

Certifications and Licesure:

  • Must be a Licensed Clinical Social Worker (LCSW) in the State of Nevada
  • Must maintain current individual malpractice policy during course of employment
  • Must have a valid driver's license.
